Updating Your Student Demographic Information

To update any of the following, complete this form and attach all supporting documentation:

  • Name Change
  • Citizenship Status
  • Date of Birth
  • Gender/Gender Identity
  • Students on a Rutgers F-1 or J-1 visa sponsorship should reach out to the appropriate campus office listed below before submitting this form for any demographic data change. The change must be aligned with the information on your passport, I-20/DS-2019 and other legal documents.
